There are several distinct types of dental X-ray, and each one exists to answer a different question. Knowing which is which helps you understand your own diagnosis, follow your treatment plan, and recognise when an image is genuinely needed versus routine.

Intraoral vs Extraoral X-Rays

The simplest way to organise dental imaging is by where the detector sits.

  • Intraoral means the sensor is placed *inside* the mouth while the X-ray source points at it from outside the cheek. These give the most detailed, high-resolution views of individual teeth and are the workhorses of everyday dentistry.
  • Extraoral means both the source and the detector are *outside* the mouth, usually on a machine that rotates around your head. These trade fine detail for a much wider field of view, capturing the whole jaw or skull.

Bitewing X-Rays (BW)

Full-Mouth Series (FMX)

A full-mouth series is not a different kind of image - it is a *complete set* of intraoral X-rays (a combination of periapicals and bitewings) that together document every tooth and its roots. It is typically taken for a new patient with a complex history, or periodically every few years, to give a comprehensive baseline. Because it involves many individual films, it is more involved than a couple of bitewings, and like all imaging it should be justified by your clinical situation rather than taken by default.

Occlusal X-Rays

An occlusal X-ray uses a larger film held between the teeth, giving a broad, flat view looking down onto (or up at) the dental arch - the roof or floor of the mouth. It captures a wider span of the jaw than a periapical.

A panoramic is the go-to for the *big picture*: assessing wisdom teeth, planning extractions, screening for cysts or large lesions, checking jaw structure, and getting an overview for a new patient. Its trade-off is detail - it is excellent for scope but not as sharp as a bitewing for spotting a small cavity between two teeth, which is why the two are often used together.

Cephalometric X-Rays

A cephalometric (or "lateral ceph") is a side-on X-ray of the whole skull and jaws. It is used almost exclusively in orthodontics and jaw surgery.

CBCT delivers far more information than a 2D X-ray - and a higher radiation dose - so it is reserved for situations that genuinely need it: implant planning (measuring bone height and the position of nerves and the sinus), complex root canals and suspected root fractures, impacted teeth close to a nerve, and jaw pathology.
